How We Support You

Our team of allied health professionals are passionate about ensuring our community have access to high quality allied health services, and in working collaboratively with medical specialists to achieve this. Consequently, we highly value and prioritise regular communication and updates to the referring medical professional following initial and final consultations, and periodically throughout the intervention period.

We guarantee that we will keep relevant stakeholders updated as regularly as necessary to ensure the optimal outcome in the client’s achievement of their goals and outcomes. Additionally, we will measure progress and monitor barriers/flags that are indicative of potential delays in recovery and achievement of their goals, and promptly communicate these concerns and relevant clinical recommendations via phone call, email updates, reports, and/or case conferences to support the client to overcome these barriers in their recovery and return to work and/or meaningful occupations.

How We Support Our Clients

Our mission at Thriving Lives Co. is to provide the knowledge, strategies, and skills to empower our community to improve their own quality of life, one step at a time, to live a life they love. To achieve this mission, we adopt a holistic, person-centred, and biopsychosocial approach in our clinical interventions with an aim to empower our clients via education and guidance to facilitate self-management of their injury and/or illness. This leads to increased confidence, motivation, and independence for the client in their own recovery, and consequential shorter recovery timeframes due to meaningful and long-term behaviour change, sustainable outcomes, and increased cost-effectiveness of rehabilitation. As part of our commitment to empowering our clients, we place a strong emphasis on focussing on function rather than disability and work hard to support our clients to remain as active, independent, and engaged in their meaningful occupations as possible.

We are an interdisciplinary team of like-minded evidence-based exercise physiologists, occupational therapists, physiotherapists, and a psychologist, which allows us to be able to support our clients in all biopsychosocial aspects of their recovery from a unified and collaborative care team to minimise the inconvenience, contradiction in information, and confusion that clients often experience when working with multiple allied health providers as part of their recovery.

Elderly individuals in need of allied health services

Via an individual’s aged care funding through My Aged Care, whether that be through AN-ACC funding, a Home Care Package (HCP), or through a Short-Term Restorative Care (STRC) Program.

Individuals living with a disability with an NDIS plan

For reasonable and necessary treatment to support their functional capacity and improvements in health and wellbeing via their NDIS support budget. To refer a client to us, you can call us or send through a completed copy of our NDIS specific referral form .

Life insurance claimants with illness and injuries

To assist them in regaining wellness and facilitate achievement of their goals, including returning to personally meaningful occupations and/or work readiness.

Injured workers for early intervention allied health services

To minimise their recovery time and increase the likelihood of their return to safe and sustainable employment. Referrals can be made via email at info@thrivinglives.com.au or fax on (07) 3539 9821 with a description of their health background and referral needs along with a copy of their most recent medical certificate.

Referrals can be made prior to or during an open and accepted workers’ compensation or income protection claim which may allow the patient to be able to receive rebates for allied health services if/once treatment is approved by the workers’ compensation scheme following your referral.

Patients living with diagnosed chronic or terminal medical condition/s

To any of our allied health team members via a general practitioner management plan for a Chronic Disease Management (CDM) plan by completion of a Medicare referral form and sending this to our administration team via email at info@thrivinglives.com.au or fax on (07) 3539 9821.

Veterans with a white and/or gold card for their accepted health conditions

Via their Department of Veterans’ Affairs (DVA) for allied health services by completion of a DVA D904 referral form and sending this to our administration team via email or fax.

Victims of motor vehicle accidents

Referrals can be made via email at info@thrivinglives.com.au or fax on (07) 3539 9821 with a description of their health background and referral needs along with a copy of their most recent medical certificate.

Referrals can be made prior to or during an open and accepted comprehensive third-party insurance claim or NIISQ claim which may allow the patient to be able to receive rebates for allied health services if/once treatment is approved by the insurance scheme following your referral.

Privately paying clients with allied health needs

Via completion of a letter outlining the client’s health background information and the purpose of the referral, which can be sent to our administration team via email at info@thrivinglives.com.au, or fax on (07) 3539 9821.

If clients possess private health cover, they may be eligible to receive a rebate so are encouraged to contact their private health fund to find out if this is included in their policy.
